Life Insurance

Life insurance provides important financial protection for you and your family. You can choose from different levels of coverage to meet your needs.

Benefits Provisions Eligibility 
Basic Life & Accidental Death and Dismemberment (AD&D) Kindred provides Basic Life and AD&D insurance for all full time employees to assist you and your family in the event of a death or dismemberment. Your coverage is effective the first day of the month following 60 days of continuous full-time employment. If the 60th day is the first day of the month then coverage is effective on that day.
Supplemental Life You can purchase additional Life insurance for yourself. You pay the full cost of any supplemental life insurance coverage. Your coverage is effective the first day of the month following 60 days of continuous full-time employment. If the 60th day is the first day of the month then coverage is effective on that day.
Supplemental AD&D You can purchase additional AD&D insurance for yourself, as well as coverage for your spouse or qualified domestic partner and your dependent child(ren). You pay the full cost of any supplemental AD&D insurance coverage. Your coverage is effective the first day of the month following 60 days of continuous full-time employment. If the 60th day is the first day of the month then coverage is effective on that day.
Spouse or Qualified Domestic Partner and Child Life Insurance

You may also purchase Life Insurance for eligible dependents. Eligible dependents include:

  • Your spouse or qualified domestic partner.
  • Your unmarried children from birth to under 26 years of age.
  • You are automatically the beneficiary.

Permanent Life In addition, you also have the option to purchase permanent life insurance. With a permanent life insurance policy, you can be the policy owner and maintain the coverage, whether or not you leave your employer, for as long as you choose to continue to pay the premium. This option offers you the ability to provide lasting protection for your family. With the purchase of an employee permanent life policy, you may also purchase additional life insurance for your eligible dependents. Your coverage is effective the first day of the month following 60 days of continuous employment. If the 60th day is the first day of the month then coverage is effective on that day.

Disability

The loss of income due to illness or injury can cause serious financial hardship for you and your family. Disability insurance can replace a percentage of your lost income (up to a maximum benefit) to help you continue paying bills and meeting your financial obligations during this difficult time. You are eligible for disability insurance upon completing 12 months of continuous full-time employment.

Short-Term Disability Short-Term Disability benefits begin after you have been disabled for 15 consecutive days. After being out of work for 15 consecutive days, you will be paid a portion of your base salary, up to a weekly maximum. Benefits end after 180 days.
Long-Term Disability Long-Term Disability benefits begin after you have been disabled for 180 days of continuous disability. After being out of work for 180 days of continuous disability, you will be paid a portion of your base salary, up to a monthly maximum.

401(k) Plan

Kindred offers a 401(k) retirement savings plan that allows you to contribute contribute pre-tax and/or after-tax (Roth) dollars through payroll deduction.

You are eligible to participate in the 401(k) plan the first of the month following one month of service, with the exception of Personal Care Attendants/Assistants or if you reside in Puerto Rico. After 12 months of service, the Company may match a percentage of the first 4% of pay contributed to the plan.
